How to use this template

Step 1: Open the Schedule of Values (G703) tab. Fill in your line items and scheduled values. Each month enter the work completed this period — the rest calculates automatically. Step 2: Copy the Total Completed value from the SOV tab into the Cover Sheet, Line 4. Step 3: Fill in the project info and application number on the Cover Sheet. Lines 1–3 pull from your project info. Line 5 (retainage) and Line 6 (total earned less retainage) calculate automatically. Step 4: Enter your previous certificates for payment on Line 7. Line 8 (current payment due) calculates automatically. Step 5: Sign the certification block and submit with your required compliance documents.

What to attach with your pay application

Most GCs require you to submit your pay application with a conditional lien waiver for the current period, a current Certificate of Insurance, and any other documents specified in your contract. Missing any of these is the most common reason pay apps get rejected.

This template is provided for general reference and educational purposes. It is not an AIA document. Requirements vary by contract — confirm with your GC what format they accept.
